  1. Private Health Information
    • By registering for this website and giving us your information, you agree that we may talk to anyone that we need to about your case, including: our staff, the contact person you list, the patient who is on the application, the doctor you list - and his/her office staff, the insurance companies you list, and employees of the pharmacy that you list. In addition, we may discuss your application with the other copayment assitance organizations to make sure that there is no duplication of payments. IF you do not agree to this - do not apply.y
    • We will NOT discuss your case with any of the pharmaceutical companies, or anyone who does not need to know. We will discuss your case with the minimum number of people required to process your claim in a fair manner.
  2. Collection of Information
    • We collect the following information as you visit our website - in addition to the information you voluntarily submit: the domain name of the website you visited right before coming to our website; your computer's IP address; the date and time of your visits to our website. We collect this information to help analyze where our visitors come from and how they interact with our website.
    • Our website uses cookies. Cookies are small text files that are stored on your computer and allow you to stay logged in. IF you block cookies on our website, you will have problems staying logged in. IF you turn off javascript, our forms might not work.
    • We do not disclose, give, sell, or transfer any personal information about our web sites' visitors unless required for law enforcement or otherwise required by law.

    • Email is not secure. It can be read by anyone who has access to the servers and routers between your computer and our computer. Never send anything secret by email.
    • We will never sell, give or trade our database of email addresses to any other person or organizations.
    • If you subscribe to our Brain Tumor News Blast, we WILL send occasional messages of interest to the brain tumor community. You can easily opt out of getting emails by following the instructions in every email on how to unsubscribe.
    • Communications about your grant to you, your doctors and their staff, or the pharmacy and their staff is much easier by email than trying to call. We tried implementing secure email solutions but too many people had problems with it so we now use unencrypted - regular - email. The information sent by email is your name, email address, status of your grant, and any messages about your grant or claims. The email does mention brain tumor treatments. We will not send your social security number. There is a small chance that these emails can be intercepted and read by unauthorized people. If that would be a problem for you - do NOT give us your email address. If you voluntary give us your email address, you are granting us permission to send email to you. If you do not want to use email, that is fine and will not interfere with the process of grant or claim approvals.
    • If you need to send us a message with private information, use our feedback form, which is encrypted.
